Hugo Landau fa4e92a70a QUIC ACK Manager, Statistics Manager and Congestion Control API
This is the initial implementation of the ACK Manager for OpenSSL's QUIC
support, with supporting design documentation and tests.

Because the ACK Manager also depends on the Statistics Manager, it is
also implemented here. The Statistics Manager is quite simple, so this
does not amount to a large amount of extra code.

Because the ACK Manager depends on a congestion controller, it adds a
no-op congestion controller, which uses the previously workshopped
congestion control API.

Pauli 16ff70a58c Remove the _fetch_by_number functions
These functions are unused and untested.  They are also implemented rather
inefficiently.  If we ever needed them in the future, they'd almost surely
need to be rewritten more efficiently.

Richard Levitte 2e4d0677ea Make it possible to remove methods by the provider that provides them
This adds ossl_method_store_remove_all_provided(), which selectively
removes methods from the given store that are provided by the given
provider.

This also adds the EVP specific evp_method_store_remove_all_provided(),
which matches ossl_method_store_remove_all_provided() but can also
retrieve the correct store to manipulate for EVP functions.

This allows us to modify ossl_provider_self_test() to do the job it's
supposed to do, but through clearly defined functions instead of a
cache flushing call that previously did more than that.

ossl_provider_deactivate() is also modified to remove methods associated
with the deactivated provider, and not just clearing the cache.

Richard Levitte 60640d79ca Don't empty the method store when flushing the query cache
When evp_method_store_flush() flushed the query cache, it also freed
all methods in the EVP method store, through an unfortunate call of
ossl_method_store_flush_cache() with an argument saying that all
methods should indeed be dropped.

To undo some of the confusion, ossl_method_store_flush_cache() is
renamed to ossl_method_store_cache_flush_all(), and limited to do
only that.  Some if the items in the internal ALGORITHM structure are
also renamed and commented to clarify what they are for.

Richard Levitte 20b6d85ab2 Drop ossl_provider_clear_all_operation_bits() and all uses of it
This is a misused function, as it was called during query cache flush,
when the provider operation bits were meant to record if methods for a
certain operation has already been added to the method store.

Hugo Landau 927d0566de Refactor OSSL_LIB_CTX to avoid using CRYPTO_EX_DATA
This refactors OSSL_LIB_CTX to avoid using CRYPTO_EX_DATA. The assorted
objects to be managed by OSSL_LIB_CTX are hardcoded and are initialized
eagerly rather than lazily, which avoids the need for locking on access
in most cases.

Fixes #17116.

Pauli af788ad6c3 fetch: convert a NULL property query to ""
Previously, a NULL property query was never cached and this lead to a
performance degregation.  Now, such a query is converted to an empty string
and cached.

Richard Levitte dc010ca6ec CORE: Encure that cached fetches can be done per provider
This mostly entails passing around a provider pointer, and handling
queries that includes a pointer to a provider, where NULL means "any".

This also means that there's a need to pass the provider pointer, not
just down to the cache functions, but also be able to get it from
ossl_method_store_fetch().  To this end, that function's OSSL_PROVIDER
pointer argument is modified to be a pointer reference, so the
function can answer back what provider the method comes from.

Test added.

Matt Caswell 59a783d05a Fix a race in ossl_provider_add_to_store()
If two threads both attempt to load the same provider at the same time,
they will first both check to see if the provider already exists. If it
doesn't then they will both then create new provider objects and call the
init function. However only one of the threads will be successful in adding
the provider to the store. For the "losing" thread we should still return
"success", but we should deinitialise and free the no longer required
provider object, and return the object that exists in the store.

Matt Caswell 36a89c0439 Init the child providers immediately on creation of the child libctx
We were deferring the initial creation of the child providers until the
first fetch. This is a carry over from an earlier iteration of the child
lib ctx development and is no longer necessary. In fact we need to init
the child providers immediately otherwise not all providers quite init
correctly.

Richard Levitte 309a78aa30 CORE: Add an algorithm_description field to OSSL_ALGORITHM
This corresponds to the |info| field in EVP_PKEY_ASN1_METHOD, as well
as the generic use of OBJ_nid2ln() as a one line description.

We also add the base functionality to make use of this field.

Rich Salz cd3f8c1b11 Always check CRYPTO_LOCK_{read,write}_lock
Some functions that lock things are void, so we just return early.

Also make ossl_namemap_empty return 0 on error.  Updated the docs, and added
some code to ossl_namemap_stored() to handle the failure, and updated the
tests to allow for failure.

Matt Caswell b574c6a9ac Cache legacy keys instead of downgrading them
If someone calls an EVP_PKEY_get0*() function then we create a legacy
key and cache it in the EVP_PKEY - but it doesn't become an "origin" and
it doesn't ever get updated. This will be documented as a restriction of
the EVP_PKEY_get0*() function with provided keys.

Matt Caswell d84f5515fa Don't hold a lock when calling a callback in ossl_namemap_doall_names
We don't want to hold a read lock when calling a user supplied callback.
That callback could do anything so the risk of a deadlock is high.
Instead we collect all the names first inside the read lock, and then
subsequently call the user callback outside the read lock.

Without this, a provider  has no way to know that an application
has finished with the array it returned earlier.  A non-caching provider
requires this information.

Pauli 299f5ff3b5 provider: add option to load a provider without disabling the fallbacks.
Add an argument to PROVIDER_try_load() that permits a provider to be
loaded without changing the fallback status.  This is useful when an
additional provider needs to be loaded without perturbing any other setup.
E.g. adding mock providers as part of unit testing.

Pauli 64954e2f34 Fix race condition & allow operation cache to grow.
This fixes a race condition where the index to the cache location was found
under a read lock and a later write lock set the cache entry.  The issue being
that two threads could get the same location index and then fight each other
over writing the cache entry.  The most likely outcome is a memory leak,
however it would be possible to set up an invalid cache entry.

The operation cache was a fixed sized array, once full an assertion failed.
The other fix here is to convert this to a stack.  The code is simplified and
it avoids a cache overflow condition.

Matt Caswell 0b07db6f56 Ensure the EVP_PKEY operation_cache is appropriately locked
The EVP_PKEY operation_cache caches references to provider side key
objects that have previously been exported for this EVP_PKEY, and their
associated key managers. The cache may be updated from time to time as the
EVP_PKEY is exported to more providers. Since an EVP_PKEY may be shared by
multiple threads simultaneously we must be careful to ensure the cache
updates are locked.

Richard Levitte 390f9bad69 CORE: Separate OSSL_PROVIDER activation from OSSL_PROVIDER reference
This introduces a separate activation counter, and the function
ossl_provider_deactivate() for provider deactivation.

Something to be noted is that if the reference count goes down to
zero, we don't care if the activation count is non-zero (i.e. someone
forgot to call ossl_provider_deactivate()).  Since there are no more
references to the provider, it doesn't matter.
The important thing is that deactivation doesn't remove the provider
as long as there are references to it, for example because there are
live methods associated with that provider, but still makes the
provider unavailable to create new methods from.

Richard Levitte 4ce1025a8a PEM: Make PEM_write_bio_PrivateKey_traditional() handle provider-native keys
PEM_write_bio_PrivateKey_traditional() didn't handle provider-native
keys very well.  Originally, it would simply use the corresponding
encoder, which is likely to output modern PEM (not "traditional").

PEM_write_bio_PrivateKey_traditional() is now changed to try and get a
legacy copy of the input EVP_PKEY, and use that copy for traditional
output, if it has such support.

Internally, evp_pkey_copy_downgraded() is added, to be used when
evp_pkey_downgrade() is too intrusive for what it's needed for.
