Richard Levitte 3c9f992aa0 DOCS: Don't mention internal functions in public documentation
This time noticed in OSSL_trace_set_channel.pod, and it turned out to
be easy to mention the public functions affected instead.

Richard Levitte 879365e6d4 Make header references conform with man-pages(7) in all manuals
Details from man-pages(7) that are used:

   Formatting conventions (general)

       ...
Filenames (whether pathnames, or references to header files) are always
in italics (e.g., <stdio.h>), except in the SYNOPSIS section, where in‐
cluded files are in bold (e.g., #include <stdio.h>). When referring to

Rich Salz fadb57e554 Fix some missing doc links.
Replace "=for openssl foreign manuals" with simpler syntax, it looks
like the "=for openssl ifdef" construct.
Fix some broken L<> links; add some missing foreign references and fixed
some typo's.
The WARNINGS in dhparam referred to non-existant commands so reword it.

Richard Levitte bb82531f65 POD: stop abusing comment
OpenSSL uses some POD directives masquerading as 'comment'
('=for comment' etc). This is abusive and confusing. Instead, we use
our own keyword.

    =for openssl whatever

    =begin openssl

    whatever

    =end openssl

(we have never used the multiline form, but might start one day)

Rich Salz 485d336137 Do not have duplicate section heads
Change find-doc-nits to complain if a section header is repeated,
within a parent header (i.e., duplicate =head2 within a =head1).
In almost all cases, we just remove the duplicate header, as
it was a "continuation" of the =head1 that was already in affect.
In some cases, just remove "=head1 NOTES", possibly moving text
around, because the "NOTES" were really important parts of the
DESCRIPTION section.

No =headX sections should end with a period.
All =head1 labels should be in all uppercase.
No sub-head (=head2, etc) should be in all uppercase.
Update find-doc-nits to reject the above.

Dr. Matthias St. Pierre 3a8269b319 trace: rename the default trace category from 'ANY' to 'ALL'
It seems more intuitive to set `OPENSSL_TRACE=all` instead of
`OPENSSL_TRACE=any` to obtain trace output for all categories.

Dr. Matthias St. Pierre 02bd2d7f5c trace: apps/openssl: print the correct category name
Previously, if the openssl application was run with OPENSSL_TRACE=any,
all trace output would just show 'ANY' as the category name, which was
not very useful. To get the correct category name printed in the trace
output, the openssl application now registers separate channels for
each category.

The trace API is unchanged, it is still possible for an application to
register a single channel for the 'ANY' category to see all outputt,
if it does not need this level of detail.

Dr. Matthias St. Pierre c37e635065 trace: update the documentation
This commit adds some missing symbols and other minor enhancements.
In particular, it establishes the term 'channel' as a synonym for
a BIO object attached to a trace category, and introduces the
concept of a 'simple' channel versus a 'callback' channel.
